40 audiometry practice cases

Explore different ages, severities and shapes of cochlear loss, conductive and mixed losses, cochlear asymmetry, cookie-bite and noise patterns, and otosclerosis-style teaching profiles. The first ten cases identify the pattern; cases 11–40 leave the interpretation to you.

Cases 1–35 open with assessment mode off, so Tutorial, Patient settings, Visual helper and Masking helper are available. Cases 36–40 are full tasks with an empty plot and assessment restrictions. Each link opens its own simulator tab; the answer remains here until you choose to reveal it.

How to practise and check masking
  1. Open a practice case. For cases that need masking, the supplied plot and PNG show not-masked AC and the initial BC measurements, which may reflect cross-hearing. They are your starting results, not necessarily each ear’s true thresholds.
  2. In Tutorial, select Not-masked thresholds and use Check current audiogram to check the starting plot where supplied. Cases without masking start with an empty plot; measure them before checking. To practise the entire test in a case with supplied results, use Delete all first. Avoid Generate and set patient: it replaces the case.
  3. Open Masking helper. Select Rule 1, 2 or 3 and add rows for the frequency and ear under test that you think need masking. Enter the plotted left/right AC and BC values and the masker start level (the opposite ear’s effective AC at that frequency). Use Check entries to check your rows and find missing requirements for the selected rule. Add missing entries supplies outstanding rows for that rule; use it after your own attempt. Repeat for all three rules.
  4. Recheck the threshold with the masker off, then perform threshold searches starting the masker at the opposite ear’s AC. Raise the masker in 10 dB steps. Establish at least four measurements, with the last three thresholds within 5 dB. If no plateau is reached before the masker output limit, document the masking limitation. Store the result with masking on. Completed worksheet rows disappear; recheck all rules as masked results can reveal further requirements.
  5. For the finished plot, choose Mask where required, with not-masked shadows in Tutorial and Check current audiogram. Use Check method to review your recorded procedure. Plot patient thresholds replaces your work with the reference plot, so use it only after finishing or after saving your evidence. Compare the pre-masking PNG with Show answer here.
  6. For assessment cases, measure the full PTA independently. Teaching tools are disabled, but Presentation log and Masking chart remain available. Reveal the answer here when finished.

Difficulty reflects the initial masking workload: Easy has no rule triggers; Medium has a smaller Rule 1/2 workload; Hard has many targets, Rule 3, or a masking limitation. These are teaching grades, not clinical severity grades. Requirements can change after masking.

Answers use Tutorial’s ideal thresholds and required masking with not-masked shadows. BC is plotted at 500, 1000 and 2000 Hz. Thresholds above the frequency-specific AC/BC output limit show no response (NR) at that limit. Response variability can change your measured results; the Tutorial check allows ±5 dB. Otosclerosis-style profiles combine a conductive slope (greater at low frequencies, still present at high frequencies) with a 2 kHz BC transmission notch. They are simulated teaching patterns, not diagnoses.
