Occlusion of large or medium pulmonary artery is usually embolic
- 95% arise from thrombi in large veins of lower legs
- Popliteal
- Femoral
- Iliac
- Depending on size the embolism may:
- Occlude pulmonary artery
- Impact astride the bifurcation
- Pass into progessively smaller branching pulmonary arteries
